WELDER CRUSHED WHEN 1,000 POUND WELDMENT FELL ON HIM

EMPLOYEE #1 WAS WORKING AT A WELDING STATION ALONE. THE WELDMENT, WHICH HE HAD ATTACHED TO THE POSITIONER, CAME LOOSE AND FELL ONTO HIM. THE 1000 POUND WELDMENT PREVENTED HIM FROM BREATHING AND HE WAS ASPHYXIATED. EMPLOYEE #1 WAS THE LEAD WELDER FOR THE DEPARTMENT, AND TRAINED OTHER WELDERS. THE WELDMENT WAS NOT ATTACHED TO THE POSITIONER WITH AN ADEQUATE QUANTITY OF CLAMPS. AS THE WELDMENT HEATED UP, DUE TO THE WELDING, IT EXPANDED. EVENTUALLY THE EXPANSION-CONTRACTION ACTION CAUSED THE WELDMENT TO LOOSEN AND FALL AND STRIKE EMPLOYEE #1.
