ELECTRIC SHOCK - DIRECT CONTACT WITH ENERGIZED PARTS

AN EMPLOYEE WAS ABOUT 5 FEET ABOVE THE FLOOR, STANDING ON A FIBERGLASS LADDER ADJUSTING THE DOWN LIMIT SWITCH ON A 240-VOLT OVERHEAD HOIST THAT WAS ATTACHED TO AN UNDER-HUNG OVERHEAD CRANE. TO DO THIS, THE EMPLOYEE HAD TO REMOVE THE HOIST'S HOUSING, EXPOSING AN ENERGIZED TERMINAL BLOCK AT THE END OF THE UNIT. AS HE WAS DEENERGIZING THE HOIST BY MEANS OF A TWIST-LOCK PLUG, THE EMPLOYEE CAUSED THE HOIST TROLLEY TO MOVE, AND HIS HEAD CONTACTED THE ENERGIZED TERMINALS. THIS CAUSED THE EMPLOYEE TO LOSE HIS BALANCE, AND HE GRABBED THE HOIST CHAIN TO BREAK HIS FALL. THE EMPLOYEE WAS HOSPITALIZED FOR BURNS ON HIS RIGHT CHEEK.
