Investigation Summary
Employees #1 through #4, subcontractors of a water company, and eight coworkers were injured when a water main break and gas main break occurred at the same time. The six gas company workers did not evacuate the water workers, and all four sustained burns that required hospitalization. The water subcontractors, who accidentally ignited the gas, had equipment that was intrinsically unsafe and many sources of ignition. The injured included one water company worker and one civilian.
