Employee fatally struck in head by flying wooden spool

At approximately 9:00 a.m. on July 17, 1993, Employee #1 was feeding Abbaca fiber rope from a wooden spool into a 4 inch pvc pipe that was attached to a mandrell and being pulled extremely hard by a backhoe on the south end of the project. The rope on the pulling end was nylon. Employee #1 positioned himself between the wooden spool and the interior of the underground precast and applied tension. The slingshot effect of the nylon rope caused the wooden spool to rapidly fly into the air, striking Employee #1 as he was bent over looking/listening into the 4 inch pvc pipe, causing a massive and fatal brain contusion.
