Employee injured in fall from structure

At approximately 9:15 a.m. on December 10, 1990, Employee #1 was working on a poured concrete structure installing braces for the next vertical pour. While Employee #1 held a brace, a coworker pulled nails out of a turnbuckle attached to the brace to shorten its length. As the nail began to loosen, the coworker alerted Employee #1 to its status and saw the employee fall to a lower level, then bounce off scaffolding. The employee fell 18 ft. The employee was wearing his safety belt and lanyard but was not tied off. The coworker had his safety belt tied off to the form. Employee #1 was hospitalized.
