Investigation Summary
Two employees of a painting contractor were engaged in sandblasting the structural steel underside of a street bridge that passed over four sets of railroad tracks. The employees were working from two adjoining sets of track that had been shut down and de-energized, so that work could proceed, while train traffic continued through the area on the remaining two tracks. There was no flagman on duty to warn the employees of approaching trains. For some reason, Employee #1 wandered from the work area into the path of an oncoming train. He was killed. It is likely that the noise created by the sandblasting operation may have drowned out the sound of the approaching train. Having a trained and competent flagman on duty to warn employees of approaching trains could have prevented this accident.
