Employee killed in fall from roof

At approximately 2:30 p.m., Employee #1 and his coworkers were installing insulation and metal roof panels on a low-sloped 1:24 roof of a metal frame building. The peak was 35 ft; the eave 30 1/2 ft; and the metal roof panels were 2 ft wide by 70 ft long. Two 4 ft wide panels had been installed when Employee #1 stepped onto the insulation and fell 32 feet to the concrete floor. He sustained head injuries and severe fractures, and was taken to the local hospital, where he died at 2:00 a.m. the following morning. The employees constructing the leading edge were not protected by a guardrail system, safety net, or personal fall arrest system.
