Employee amputates fingers while operating press brake

On September 30, 2004, Employee #1 was operating a press brake of the DR assembly machine when she reached into the point-of-operation. The rotating parts crushed four fingers on her left hand, which later were amputated at a medical center. She was hospitalized for her injuries.
