EMPLOYEE STRUCK AND KILLED BY LARGE PIECE OF ELEC. EQUIPMENT

EMPLOYEE #1 WAS SUPERVISING THE UNLOADING OF LARGE PIECES OF ELECTRICAL EQUIPMENT CALLED C.T. CABINETS. THESE CABINETS, MADE BY SQUARE D, ARE LARGE, BULKY, TOP HEAVY OBJECTS WEIGHING BETWEEN 1,000 AND 2,500 POUNDS. A CASE W20C WHEELOADER FORKLIFT WAS BEING USED TO UNLOAD THE CABINETS FROM THE BACK OF A 28 FOOT TRAILER. WHEN THE THIRD CABINET WAS BEING UNLOADED IT FELL OFF THE FORKS AS IT WAS BEING LOWERED. IT STRUCK AND PINNED EMPLOYEE #1 TO THE FLOOR OF THE TRAILER. HE SUFFERED A FATAL SKULL FRACTURE.
