Employee crushed and killed against beam

Employee #1 was using a forklift truck to remove boxes from a pallet rack that was 173 in. high by 33 in. deep by 145 in. wide. The space between the racks where the employee was working was 85 in. wide. As he lowered a 550 lb load to ground level, he backed up between the pallet racks into a horizontal support beam. He was killed when he crushed himself against the beam while standing inside the forklift truck.
