Employee dies of head injuries after falling from roof

At approximately 12:40 p.m. on May 8, 1991, Employee #1 was installing insulation from a roof that was approximately 20 feet above the ground when he fell through the roof hole, striking his head on a tile floor. He died from crushing blunt force head injuries.
