Employee killed in fall from elevated pallet

Employee #1 was demolishing structural steel while elevated 10 ft on a pallet inserted on the forks of a moving lift truck. All supporting bolts of a 14 ft horizontal I-beam assembly had been flame cut when the truck and elevated platform moved forward, striking the I-beam assembly. Both the I-beam and the employee fell. The employee sustained fatal skull crushing and brain puncture injuries. The elevated work platform was not safeguarded by protective railing or the use of a lift truck safety platform.
