Employee killed when head is crushed by lift mechanism

As Employee #1, a punch press set-up person, stood by a battery-powered portable die lift cart, he stuck his head into an opening in the cart mast and accidentally engaged the lift mechanism. He was killed when his head was crushed between the rising lift mechanism and the top of the horizontal mast support.
