Employee killed when struck by falling rack

At approximately 1:00 p.m. on August 13, 1988, Employee #1 was found on the floor with severe body and head injuries, from which he died. There were no witnesses to the accident. It is assumed that he was using a Towmotor to push a cantilever steel rack that was approximately 8 ft wide by 15 1/2 ft long and weighed approximately 2,500 lb. Near the east exit door, the rack became askew and, because there was not enough clearance to get the Towmotor to the other side of the rack to straighten it, one end of the rack was raised by a crane. The Towmotor moved to the other end and while the rack was being lowered to the floor, the rack became disengaged from the hook of the steel chain sling and struck Employee #1.
