EMPLOYEE CRUSHED BETWEEN PALLET CARS

Employee #1 a supervisor was between two mold pallet cars trouble shooting a haul back device which was located in the center of the mold pallet car track system. He straddled the haul back device with his back to the mold pallet cars. Another employee noticed the cars began to move and activated the emergency stop however, the cars continued to move. Employee # 1's leg was caught preventing his escape and he was fatally crushed. A lockout procedure had been developed and all employees trained, but procedures were not followed.
