EMPLOYEE KILLED WHEN STRUCK IN HEAD BY FALLING ANODE

EMPLOYEE #1 WAS IN A SQUATTING POSITION, REMOVING TAPE FROM AN ENGINE CYLINDER LINER. ANOTHER EMPLOYEE PLACED A LEAD ANODE, IN AN UPRIGHT POSITION, ON THE FLOOR NEAR EMPLOYEE #1'S POSITION. A THIRD EMPLOYEE ATTEMPTED TO DRIVE A FORKLIFT BETWEEN THE LEAD ANODE AND A BAFFLE STORAGE BOX. THERE WAS INSUFFICIENT ROOM FOR THE FORKLIFT AND THE REAR OF THE LIFT STRUCK THE LEAD ANODE, KNOCKING IT OVER IN THE DIRECTION OF EMPLOYEE #1. THE ANODE STRUCK EMPLOYEE #1 IN THE HEAD, KILLING HIM.
