Event: 05/13/1992
Employee dies of asphyxia in trench cave-in

The employees were replacing a water line. At the time of the collapse, the employees were finishing the job at the deepest point of the trench, at approximately 9 ft. The trench was unshored and unsloped and approximately 8 ft wide at the point of collapse. During the week that preceded the accident, water had to be pumped from the bottom of the trench each morning. A Case 580 backhoe was in use that day. A cave-in occurred, burying Employee #1 up to his waist. He died of asphyxia. A second cave-in buried Employee #2, who escaped injury.
