Employee killed when run over by logging truck

At approximately 3:00 p.m. on November 18, 1992, Employee #1, a loader/dozer operator, was helping a skidder operator pull a loaded log truck to firmer ground with a winch and chain. The skidder operator began pulling the truck at approximately 3 to 5 mph. Employee #1 was walking down the driver's side of the log truck approximately 3 ft from the truck. The log truck driver stated that he had been pulled approximately 30 ft when he observed Employee #1 in his driver's side mirror, lying on the ground. The log truck driver blew his horn, and the skidder operator stopped pulling. The rear tires on the driver's side of the logging truck had rolled over Employee #1's neck and head, killing him.
