Employee drowned in papermill effluent clarifier tank

Employees were pouring a slab between the east and west papermill effluent clarifier tanks and had tasks to perform on the north and south ends between the tanks. The only safe way to get from the north end to the south end was to walk around the tanks. As a shortcut, employees walked on the pipes, cable trays, and a 6 in. tank wall to get from one end to another. Employee #1 apparently was taking the shortcut when he fell into the east clarifier, which was full of mill effluent. He drowned. No one saw Employee #1 fall into the clarifier. His body was recovered two days later when the tank was drained.
