Employee killed in fall from girder

Employee #1 was engaged in highway construction on an overpass 40 ft above ground level. A two-man crew was using a crane to position pre-stressed 4 ft by 8 ft concrete panels, called stay-in-place panels, between the concrete girders. One panel did not line up correctly with the girders and the other panels. Employee #1 was using a pry bar to position the panel while standing on the edge of the girder. He was not wearing a safety belt, and nets or other means of fall protection had not been provided. He lost his balance, slipped over the edge of the girder, and fell 40 ft to his death.
