EMPLOYEE KILLED WHEN STRUCK BY SKID BEING LOADED ONTO TRUCK

AT APPROXIMATELY 10:30 AM ON DECEMBER 21, 1987, EMPLOYEE #1 WAS HELPING ANOTHER EMPLOYEE PLACE A 12 FEET, 2 INCH BY 4 FEET, 4 INCH SKID IN AN UPRIGHT POSITION ON A 15 FOOT HIGH, 10 FOOT DIAMETER METAL TANK. THE TRUCK DRIVER WAS PREPARING TO BACK UP THE TRUCK SO THAT THE TOP PORTION OF THE SKID WOULD BE ALONGSIDE THE TRUCK. PART OF THE SKID WAS ON THE GROUND WITH THE SKID'S LIP ON THE BOTTOM OF THE TANK. WHEN HE WAS LAST SEEN ALIVE, EMPLOYEE #1 WAS STANDING BY THE RIGHT REAR WHEEL OF THE WINCH TRUCK. THE DRIVER TOLD EMPLOYEE #1 TO STAY CLEAR AND NOT TRY TO RELIEVE THE SLACK ON THE WINCH. THE DRIVER KICKED THE WINCH OUT OF GEAR, PUT THE TRUCK IN REVERSE AND RELEASED THE PARKING BRAKES. WHEN A NOISE WAS HEARD, THE DRIVER TURNED AROUND AND SAW THE SKID ON THE TRUCK BED. EMPLOYEE #1 WAS LYING BESIDE THE SKID, DEAD FROM HEAD INJURIES.
