Investigation Summary
Employee #1 was setting up an oil rig in a manufacturing yard for testing. He dropped into a sitting position to move from a section of the rig that was approximately 12 ft above ground level onto a scaffold plank at the same elevation. The jarring created by his drop onto the plank caused it to twist and come loose, which, in turn, caused the employee to fall backward to the concrete and land on his head. The plank also fell, striking him in the back. Employee #1 was hospitalized.
