EMPLOYEES BURNED WHEN PHOSPHOROUS WAS SPLASHED ON THEM

EMPLOYEES #1 AND #2 WERE ASSIGNED THE TASK OF REPLACING A FLANGE AND A VALVE IN THE P4 STORAGE TANK AREA. EMPLOYEE #2 WAS WEARING A NOMEX SUIT BUT EMPLOYEE #2 WAS NOT WEARING ANY TYPE OF PPE. THE PLAN WAS FOR EMPLOYEE #2 TO DO THE ACTUAL WORK WHILE EMPLOYEE #1 STOOD BY WITH A HALF INCH WATER HOSE IN CASE ANY PROBLEMS DEVELOPED. A NIPPLE CONNECTION ON THE LINE BEHIND EMPLOYEE #1 SUDDENLY RUPTURED, SPRAYING THE AREA WITH PHOSPHOROUS. THE NIPPLE WAS 3 INCHES LONG. EMPLOYEES #3 AND #4, WERE ALSO IN THE AREA AND WERE SPRAYED WITH PHOSPHOROUS. SINCE PHOSPHOROUS IGNITES WHEN IT COMES IN CONTACT WITH AIR, THE SPRAYED EMPLOYEES SUFFERED MINOR BURNS. EMPLOYEE #3 ALSO SUFFERED A CUT ON HIS ELBOW AS HE WAS SCRAMBLING TO GET AWAY FROM THE SPRAYING PHOSPHOROUS.
