FINGER AMPUTATED IN METAL BAND SAW

EMPLOYEE #1 WAS OPERATING AN AUTOMATIC HYDRAULIC BAND SAW, USING IT IN THE MANUAL MODE. HE REVERSED THE BLADE DIRECTION, BUT FORGOT TO SHUT THE SAW OFF. HE REACHED INTO THE DANGER ZONE TO MEASURE THE DEPTH OF THE CUT AND GOT HIS GLOVE CAUGHT IN THE RUNNING BLADE. HIS RIGHT INDEX FINGER WAS AMPUTATED AT THE FIRST JOINT.
