EMPLOYEE #1 WAS USING A TABLE SAW TO CUT A SLOT IN A PIECE OF WOOD. THE SAW BLADE FLIPPED THE PIECE OF WOOD UP, CAUSING THE EMPLOYEE'S FINGERS TO COME INTO CONTACT WITH THE REVOLVING SAW BLADE.
