FINGERTIPS AMPUTATED IN PRESS

EMPLOYEE #1 HAD JUST INSERTED A PRINTED CIRCUIT BOARD INTO THE PRESS. BEFORE SHE COULD GET HER HANDS OUT OF THE POINT OF OPERATION, THE MACHINE TRIPPED OR CYCLED. THE PULL BACK DEVICE FOR THE 60 TON JOHNSON, SINGLE STROKE, FULL REVOLUTION PUNCH PRESS, SERIAL #59393, WERE NOT ADJUSTED TO PULL HER HANDS FAR ENOUGH BACK. TTHE OPENING WAS 8 INCHES BY 16 1/2 INCHES BY 4 1/2 INCHES DEEP. THE PRESS HAD A FOUR INCH STROKE. THE EMPLOYEE LOST THE TIP OF HER MIDDLE FINGER, JUST ABOVE THE KNUCKLE, AS WELL AS THE TIP OF HER INDEX FINGER. THE BONE IN THE TIP OF HER RING FINGER WAS FRACTURED. THESE INJURIES WERE TO FINGERS ON HER LEFT HAND.
