CONSTRUCTION EMPLOYEES THROWN AND CRUSHED BY MACHINE BUCKETS

EMPLOYEES #1-3 WERE REPAIRING AND REPLACING WORN AND BROKEN DRIVE CHAINSAND BUCKETS IN AN EXTRACTOR MACHINE. AS THE EMPLOYEES WERE JOGGING THE DRIVE CHAIN, THE SHEAR PINS BROKE, CAUSING THE BUCKETS TO DROP 10 FEET. A LOOSE BUCKET FELL ON EMPLOYEE #1, CRUSHING HIM. EMPLOYEE #2 WAS THROWN OUT OF A BUCKET AND CAUGHT HIS LEG BETWEEN A BUCKET AND STEEL BEAM. EMPLOYEE #3 WAS THROWN DOWN THE UNLOAD HOPPER CAUSING BUMPS AND BRUISES. EMPLOYEE #1 WAS FATALLY INJURED. EMPLOYEES #2 AND #3 WERE HOSPITALIZED.
