EMPLOYEE FELL FROM STEPLADDER; STRUCK HEAD ON FLOOR; KILLED

EMPLOYEE #1 WAS WORKING FROM THE FOURTH STEP OF A SIX FOOT HIGH STEPLADDER. THE STEPLADDER WAS BEING USED IN A CLOSED POSITION, LEANING AGAINST FRAMEWORK, LEGS BRACED AGAINST A RAISED CONCRETE PORTION OF THE FLOOR. THE STEPLADDER WAS ALSO BEING HELD IN POSITION BY ANOTHER EMPLOYEE. WHEN EMPLOYEE #1 REACHED UP TO "TOENAIL" A VERTICAL 2" BY 4" MEMBER INTO THE FRAMEWORK, HE LOST HIS BALANCE AND FELL BACKWARDS, STRIKING HIS HEAD ON THE CEMENT FLOOR. HE SUSTAINED HEAD INJURIES, RESULTING IN HIS DEATH.
