Investigation Summary
A PIECE OF STEEL BEING HYDRAULICALLY PUSHED FROM A LARGE SHEAR MACHINE (WEIGHING APPROXIMATELY 500 TONS) BECAME CAUGHT ON A GUARD SHIELD WHICH WAS NOT SECURED TO THE MACHINE. THE GUARD TOPPLED ONTO TWO EMPLOYEES WHO WERE SORTING SCRAP METAL CUT BY THE SHEAR. EMPLOYEE #1 WAS CLOSE TO THE SHEAR (AT THE WEST SIDE OF THE SHAKER SORTER TABLE) AND RECEIVED MULTIPLE FATAL INJURIES. EMPLOYEE #2, ON THE EAST SIDE, WAS ALSO STRUCK BY THE HEAVY (APPROXIMATELY 3 TONS IN WEIGHT) SHIELD, BUT WAS THROWN TO THE SIDE OF THE TABLE AND RECEIVED NON-FATAL LACERATIONS TO HIS RIGHT LEG. THE SHIELD HAS NOW BEEN SECURED TO THE MACHINE TO PREVENT ANY RECURRENCE OF THIS TYPE ACCIDENT.
