HOIST OPERATOR STRUCK AND KILLED BY FALLING MOLD

A FORKLIFT-TYPE HOIST, FIXED IN PLACE, WAS BEING USED TO PLACE POURED MOLDS INTO THE "SHAKEOUT" IN THIS GRAY IRON FOUNDRY. THE LOAD ROTATOR FAILED, CAUSING THE PALLET TO TILT. THE MOLD SLID OFF THE PALLET AND STRUCK AND KILLED THE HOIST OPERATOR.
