EMPLOYEE CRUSHED BETWEEN PALLETS OF INGOTS AND LUMBER

EMPLOYEE #1, AGED 19, WAS FATALLY INJURED WHILE WEATHERPROOFING STACKS OF LUMBER FOR EXPORT. HE APPARENTLY STEPPED ON AN EXTENDED INGOT (OF A PALLET OF INGOTS) WHICH WAS LOCATED ADJACENT TO A PALLET OF LUMBER. THE INGOT PALLET TOPPLED, PINNING THE EMPLOYEE BETWEEN THE 1,800 POUND PALLETS OF INGOTS AND LUMBER. HE WAS KILLED. THE EMPLOYEE FAILED TO USE THE LADDER WHICH THE EMPLOYER HAD FURNISHED FOR SAFELY ACCOMPLISHING THE ASSIGNED TASK.
