Employee killed when head strikes truck cab wheel

Employee #1 was a member of a crew that was setting up an asphalt plant for a road paving job. One part of the asphalt plant was the cold bin feed, consisting of three large bins on a truck trailer. After the trailer was disconnected from the truck cab, a laborer drove the cab out until the fifth cab wheel was exposed. He then parked the cab and set the emergency brake. Employee #1 got on the nose of the trailer, about 3 1/2 ft off the ground, to feed 3/4 in. chain through the lifting eyes on the nose. The chain would connect the trailer to a loader, which would lift the front end of the trailer about 1 ft off the ground to extend the trailer's legs. The loader was parked on the south side of the nose with its gear in neutral and the emergency brake engaged. Employee #1 was hooking the trailer to the loader when he lost his balance and fell backward. He did not fall off the nose platform, but his head struck the fifth wheel of the truck cab. Employee #1 suffered a concussion and was killed.
