Employee killed in fall from roof

Employee #1 was bolting an air cylinder to a damper on an approximately 43 ft high roof of a wastewater treatment building. The work area was approximately 3 ft from the edge of the roof. Employee #1's wrench slipped from the damper cylinder rod, causing him to fall to the ground. He was killed. Neither guardrails nor other safeguards were in use at the time of the accident.
