Investigation Summary
On October 31, 1994, Employee #1 was form stripping from below a new bridge deck. He was standing on a temporary walkway that was constructed with the forms he was removing. As he walked across the forms, one of the plywood sheets shifted and he fell approximately 34 feet to the ground. He died.
