Two employees killed in trench cave-in

At approximately 10:10 a.m. on April 3, 1990, Employees #1 and #2 were carrying a 20 ft length of 6 in. PVC pipe along a 36 ft long by 2 ft wide by 9 1/2 ft deep trench. A 16 ft section of the east side of the trench, from the south end to a backhoe, caved in. Both employees were pinned against the west bank and completely buried. They sustained multiple fractures and were killed. The trench, which had been dug in 10 1/2 ft of fill dirt, was not shored, benched, or sloped.
