Employee killed in fall while cleaning skylight

Employee #1 was cleaning the glass panes of a tenth floor ceiling skylight while standing on the frame. He either slipped off the side frame or tripped and fell through one of the glass panes and landed on a plywood floor, which was on the sixth floor, a distance of about 60 ft. Employee #1 was killed.
