Employee killed when caught between table and machine

At approximately 11:10 p.m. on September 7, 1989, Employee #1, a computer checker, was called to the cut order machine computer terminal to replace the terminal operator while he took a short break. When she got to the station, Employee #1 began to print tickets and place them on rolls of carpet in the area. At the same time, a forklift driver was placing rolls of carpet by the tickets. The cutting machine operator was at the machine station and had lowered a new roll of carpet to the cutting table from the storage table, but the roll did not drop to the belt table. The machine operator left the station to push the roll onto the table. As she pushed the roll, the lowering device pneumatically closed. Employee #1, who was underneath the storage table placing tickets on rolls, became caught between the storage table and the lowering device. She was killed.
