EMPLOYEE'S FINGERS INJURED WHILE ADJUSTING POWER PRESS

THE OPERATOR OF A MECHANICAL POWER PRESS (NIAGARA PART REVOLUTION, 110 TON CAPACITY, MODEL #F-110, SERIAL #48894, COMPANY #T-1321) STARTED THE PRESS FOR A NEW OPERATION. THE COMPRESSED AIR JETS WOULD NOT BLOW PARTS OUT OF THE DIE. EMPLOYEE #1,A SET-UP OPERATOR, ARRIVED TO MAKE THE NECESSARY CORRECTIONS. A LIGHT CURTAIN IN THE FRONT AND A POINT OF OPERATION GUARD ON THE RIGHT SIDE WERE PROVIDED FOR EMPLOYEE PROTECTION. THE POINT OF OPERATION GUARD WAS IN THE UP POSITION DURING THE TIME EMPLOYEE #1 WAS MAKING ADJUSTMENTS. THE OPERATOR STARTED AND STOPPED THE PRESS TWICE WHILE EMPLOYEE #1 MADE ADJUSTMENTS. THE OPERATOR RETURNED TO THE CONTROL PANEL FOR A THIRD TIME AND PUSHED THE START BUTTON TO CYCLE THE PRESS. THIS TIME EMPLOYEE #1 REACHED INTO THE DIE TO MAKE ANOTHER ADJUSTMENT. HE RECEIVED A LACERATION TO THE TIP OF HIS INDEX FINGER AND AN AMPUTATION OF THE LITTLE FINGER ON HIS LEFT HAND.
