CONSTRUCTION EMPLOYEE KILLED WHEN STRUCK BY FALLING FORM

AT APPROXIMATELY 3 PM ON DECEMBER 7, 1987, EMPLOYEE #1, A GENERAL CARPENTER FOREMAN, WAS SUPERVISING A SEVEN MAN WORK CREW AT THE I75 AND SR84 DETOUR ON ROUTE I595. A 40 FOOT LONG FLATBED TRAILER WAS BEING USED TO TRANSPORT A TEN FOOT METAL FORM AND TWO FIVE FOOT SECTIONS TO ANOTHER LOCATION ON THE SITE. THE TEN FOOT FORM WAS BOLTED TOGETHER AND LASHED TO THE TRUCK, BUT THE TWO FIVE FOOT SECTIONS WERE NOT SECURED IN ANY WAY. EMPLOYEE #1 WAS WALKING ALONGSIDE THE MOVING FLATBED WHEN IT ENTERED A ROUGH AREA, JARRING THE LOAD. ONE OF THE FIVE FOOT SECTIONS FELL OFF THE TRAILER AND STRUCK HIM IN THE HEAD, KILLING HIM.
