Employee killed in fall from stepladder onto concrete floor

At approximately 10:30 a.m. on September 6, 1990, Employee #1 was installing a new hose and spray nozzle on a car. He was working from a 10 ft wooden stepladder when he fell, striking his head on the concrete floor. Employee #1 was killed.
