STRUCK BY FALLING LOAD OF STEEL BAR JOISTS

ON SEPTEMBER 22, 1986, AT APPROXIMATELY 2 PM, EMPLOYEE #1 WAS WORKING ON A TRAILER LOAD OF STRUCTURAL BAR JOISTS. HE WAS FLAGGING THE LIFT OPERATOR TO-OFF LOAD BUNDLES OF JOISTS. THREE LIFTS HAD BEEN COMPLETED. AFTER POSITIONING THE FOURTH LOAD, THE EMPLOYEE MOVED TO A SAFE POSITION ON THE CAB OF THE TRACTOR. AS THE LOAD WAS LIFTED, EMPLOYEE #1 NOTICED A PROBLEM WITH THE LOAD. HE CLIMBED BACK ONTO THE TRAILER TO ADJUST THE LOAD. BEFORE THE ADJUSTMENT COULD BE ATTEMPTED THE BUNDLE STRAPS CAME LOOSE, ALLOWING THE LOAD TO TUMBLE. EMPLOYEE #1 JUMPED TO THE GROUND ON THE SIDE OF THE TRAILER OPPOSITE THE LOAD. HE WAS STRUCK BY ONE OF THE FALLING BAR JOISTS. THE ACCIDENT COULD HAVE BEEN CAUSED BY AN OVERLOAD CONDITION OF THE LIFT TRUCK OR THE FACT THAT THE BUNDLES OF JOISTS WERE POORLY BUNDLED BY THE MANUFACTURER. THE EXACT CAUSE OF THE ACCIDENT HAS NOT BEEN DETERMINED. EMPLOYEE #1 WAS KILLED.
