EMPLOYEE #1, A CONSTRUCTION WORKER, WAS WELDING ON THE AIR CYLINDER OF A 1980 LANDOLL LOWBOY TRAILER, ID NO. A2801042 MODEL 350-J2. THE TRAILER'S FRONT WHEELS WERE PUSHED INTO THE FORWARD POSITION. THE CAM FOLLOWERS CAME OFF THE TRACK, RELEASING THE AXLE. THE AXLE FELL ONTO EMPLOYEE #1, CAUSING FATAL HEAD AND CHEST INJURIES. THERE APPEARED TO BE A DEFICIENCY IN THE MANUFACTURE OF THE TRAILER.
