Employee killed when crushed by collapsing ductwork

Employee #1 was demolishing a suspended plaster ceiling, above which was a massive section of HVAC ductwork. He was directly below the ductwork, removing the support pipe for an air handling unit that was obstructing his demolition activities. The HVAC ductwork unexpectedly collapsed onto him and killed him. The employer had not performed an engineering survey to determine the structural integrity of the ductwork before work had begun.
