Employee killed when crushed by blender

Employee #1 and coworker #1 were adding ingredients to a mix in a blender while coworker #2 was setting up tables to weigh and stamp 50 lb bags and coworker #3 was opening the bags. It was not yet time to start another mix, so they began to clean the blender and work area. Coworker #1 turned off the blender's power and coworker #2 walked 43 ft to turn off the power at the disconnect point. Coworker #1 then asked which crew member would clean the blender. Coworker #3 volunteered, but coworker #2 stopped him and said that he would do it. Coworkers #1 and #2 checked to make sure the blender was off. After taking the scraper downstairs, coworker #1 returned to the platform and noticed that Employee #1 had climbed into the blender and was sweeping it out. Coworker #1 told Employee #1 to get out of the blender. While Employee #1 was attempting to get out, coworker #2 inadvertently turned on the blender instead of the liquifier. Employee #1 was crushed and killed.
