Employee killed when struck by forms being off loaded

Employee #1 and a coworker were rigging forms to be off loaded from the bed of a truck by crane. When the crane operator lifted the load of forms in the air, the forms fell, striking Employee #1 in the head. The forms and Employee #1 fell to the ground. Employee #1 was crushed to death under the forms.
