CONSTRUCTION WORKER KILLED WHEN STRUCK BY FORKLIFT

EMPLOYEE #1 WAS WALKING APPROXIMATELY 5 FEET TO THE RIGHT REAR SIDE OF A GRADALL/LOED 534 B-8 MATERIALS HANDLER (FORK LIFT) OPERATED BY AN EMPLOYEE OF ANOTHER EMPLOYER. THE FORKLIFT OPERATOR WAS RELOCATING 20 TO 35 PIECES OF PVC PIPE (20 FEET LONG AND 4 INCHES IN DIAMETER) FOR THE PLUMBING AND HEATING SUB-CONTRACTOR. EMPLOYEE #1'S HEAD WAS STRUCK BY THE GRADALL'S RIGHT REAR TIRE AS THE OPERATOR MANEUVERED BETWEEN A PARKED VEHICLE ON THE LEFT AND TWO STACKS OF LUMBER ON THE RIGHT. THE GRADALL HAS 90 DEGREE REAR WHEEL POWER STEERING. IT WAS CARRYING THE PVC PIPE WITH THE FORKS RAISED APPROXIMATELY 4 FEET. THERE WERE NO WITNESSES TO THE ACCIDENT.
