EMPLOYEE IN A CONTAINER FELL FROM ELEVATED FORK LIFT FORKS

EMPLOYEE #1 WAS WORKING FROM A SHIPPING CONTAINER INSTEAD OF A SAFETY PLATFORM ON THE FORKS OF A FORKLIFT. THE FORKS TILTED FORWARD, ALLOWING THE UNSECURED CONTAINER TO FALL. EMPLOYEE #1 FELL OUT OF THE CONTAINER 14 FEET TO THE CONCRETE FLOOR. THE 190 POUND CONTAINER THEN FELL ONTO THE EMPLOYEE'S BACK AND HEAD, KILLING HIM.
