Employee killed when pinned between glass packs

Employee #1 was retrieving scrap wood from the top of stored cases of glass, each of which weighed 4,000 lb. Four unsecured cases tipped over, pinning him against other cases of glass. The cases pressed against his chest and he died of asphyxia.
