EMPLOYEE KILLED WHEN SCALDED BY TANK OF HOT WATER

At 8:20 a.m. on September 28, 1989, Employee #1 was removing a hose from a tank full of hot water heated to approximately 190 degrees Fahrenheit. The valve handle, which identified the condition of the valve being either in the open or closed position, had been installed in the reverse position. Employee #1 disconnected the hose thinking that the valve was closed, when instead it was opened. He was splashed with hot water and received second and third degree burns on 65 percent of his body. He was hospitalized and died a month later, on October 28, 1989, from the injuries received.
