Employee killed when boom of loader falls

Employee #1 was working on an Allis-Chalmers TL16 front end loader. He apparently raised the boom so that it would not be in the way when he aligned and secured the side panel. By the position of his body, Employee #1 was probably standing on the ladder when he reached in, started the loader, then pulled back on the boom lever. As the boom rose to its maximum height (approximately 9 ft), the lever was supposed to automatically go back to neutral and the boom was to remain in place. The hydraulic line leading to the hydraulic jacks that push out the rams that lift the boom burst before enough pressure had built up to disengage the boom lever. When the hydraulic line burst, the boom fell onto Employee #1. He was scissored between the boom arms and the body of the front end loader. Employee #1 was killed.
